Если снести акк и настроить заново - с чистой pst'шкой - то-же самое? Если да - то забываем про pst и копаем в сторону самого аутлука.

Апдейты/патчи никакие не ставились? Если примерно помните, когда началось - глянуть в event log'e перед этим - может что-то установилось.
Кстати, при запуске аутлука - в event логе никаких предупреждений не появляется?
